Custom Playbook with Formation Sub issue

I took the Steelers playbook and added about 14 new formations with Madden Playbook Merger Version 2.0.
To add all the new formations, I used the “Edit Names & Grouping” to group the right formation together. I did not compress the playbook, just click save.

Open the custom playbook I just made with Madden Playbook Editor 1.2.
Click on the Formations TAB to rearrange the new formation to be in the right order. Save the file.

I took the Balance D playbook and added about 5 new formations with Madden Playbook Merger Version 2.0.
Before I added the new Defense Formation, I had to edit the Playbooks.
A. Open the playbook in the db editor and go into the formation audible table (PBFM). Anywhere you see a "511", change it to zero.
B. Go to the PSAL table and export it to a .csv file. Open in excel and sort the values by "code" delete all lines that contain code=255. (This is some kind of placeholder code that pacfan's editor doesn't recognize. It won't affect any of the plays.)

The issue is when I used both Playbooks.
When I do formation subs on some of the defense formation, that player would also show up on the Offense.

Example:
4-3 Normal formation, I swap the FS and SS. If I go to the Single back Tight Slots formation. The FS is playing center and the SS is playing right guard. If I reset the Single back Tight Slots formation and go back to 4-3 Normal formation. The FS is playing FS and SS playing SS.

When I’m using a default Defense Playbook, I don’t have this issue.
Is this something I can fix or do I need to remove the custom defense playbook?

I often heard about problems with subs and custom playbooks, a way to avoid these is using packages instead of formation subs

I was having this issue so I tried it with stock playbooks. Even with the ATL and Balanced D playbooks, it puts players from offense on defense and vice versa. All this on top of the formation subs in the team management not being the playbooks I have set in coach options. Did anyone ever figure this out?
